IN a previous paper," the authors reported that there can be threc approaches for surgical treatment of intrahepatic gallstones. One is choledochotomy followed by extended exploration of the bile ducts up to the hepatic hilus, which is a basic method to be employed initially in all cases of common duct exploration. The second route is transhepatic cholangiolithotomy by an incision of the liver parenchyma, and the third method is liepatic lobectony which may be a better definitive proce-dure in selected cases. In addition to such litlıotomy procedures, the arıthors stressed the need of drainage operations to prevent bile stasis and recurrence of calculi. In the present report, a reevaluation will be given of the surgical treatment for intrahcpatic gallstones, based on 46 patients treated in the surgical department of Tolhoku University Hospital, Sendai, Japan.
